Community Link Worker
Katherine Hill is our Community Link Worker. She is based in our Practice every Monday and Tuesday.
Our Community Link Worker is a member of the GP Practice Team and provides advice and support on any social issues affecting you, local services and community-based groups and activities.
Katherine can support you with any social issues that are affecting your health and wellbeing, for example:
